Our head/ brain is a closed compartment ie there is no direct communication with the outside world.
When an injury causes this communication, it is called open head injury.
A big injury on the head where one can see the brain matter from outside is obviously an open injury but it is not the only type of an open injury.
Quite often people can have brain injury leading to fracture of base of skull which is not seen and through it CSF(fluid in and around brain and spinal cord) can flow out via nose or ear .This also is an open head injury as there is a communication between brain and outside environment.
The significance is that in open injuries, infection can travel from outside via this communication and enter the brain.
